First Visit
Your initial visit is generally one of information gathering and treatment planning usually lasting about an hour. Depending upon your needs, a second appointment may be suggested to discuss the findings and any necessary treatment. Dental care will be performed at subsequent visits unless emergency treatment is necessary. We will make every attempt to schedule your dental care as efficiently as possible.
Hours:
Monday, Wednesday,and Thursday: 8:00 AM to 4:30 PM; Tuesday: 8:00 AM to 2:00 PM
Please assist us by providing the following information at the time of your examination:
- Your completed Patient Information Form and Medical, Dental Information Form, along with a list of medications you are presently taking.
- If you have dental insurance, please bring your insurance information including the plan number. This will save time and allow us to help you process any claims.
IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.
X-Rays
In order to thoroughly evaluate your oral health, we need current intraoral radiographs (x-rays). If such films are obtainable from your prior dentist and are less than five years old, please furnish us with the dentist’s name and contact information, and we will make arrangements to obtain a copy for our records. Any supplemental x-rays necessary can be taken digitally at our office.
